Job Description

Job Summary

The Tree of Life Casework DPM is responsible for managing the multidisciplinary approach to care for each child, overseeing the implementation of Family Legacy Care Plan processes and procedures. This person serves as the point-person on all care plan processes and coordinates all relevant departments for various care-plan related meetings.

Specific Tasks

1. People Management

  • Manage all Caseworkers and oversee their caseload.
  • Facilitate and lead meetings in collaboration all relevant departments for various care-plan related meetings.
  • Attend regularly scheduled meetings with supervisor.
  • Ensure proper adherence to the policies and procedures of the Care Plan as stipulated by government officials, as well as the organization.
  • Delegate tasks effectively to departments/Care Plan Committee Members on all administrative, logistical, and other overall programmatic assignments.

2. Communication & Reporting

  • Communicate information on behalf of the Care Plan Committee across ministry departments and government agencies as necessitated.
  • Communicate to relevant staff and stakeholders when a child is admitted, is discharged, or is reintegrated back home; work in collaboration with Social Services.
  • Collaborate with Social Services, specifically Community Development in regard to the Parents/Guardians/home situation.

3. Event Planning

  • Maintain scheduling and venue procurement for all care-plan related meetings, such as Treatment Plan meetings, Reintegration Forecasting Meetings, etc.
  • Regularly remind Care Plan committee of deadlines for submissions of child Care Plan assessment forms.
  • Coordinate with Care Plan committee members in planning and implementation of meetings, timelines, etc.
  • Review feedback of event success assessment.
  • Give coaching and feedback to team for future events.
  • Provide feedback to supervisor regarding events.

4. Data Management

  • Ensure collection, organization, and maintenance of Child Care Plans.
  • Maintain up-to-date versions of all Care Plan documents and forms.
  • Maintain the Care Plan Google Calendar and manage all event invitations and/or reminders for various Care Plan related timeline events.
  • The Employee shall also be expected to undertake any responsibilities as may be delegated by his or her supervisor.

2. All other Tree of Life departments – Clear concise and timely communication to all affected departments on programs and activities involving children.

3. Other Family Legacy departments – coordinate details for applicable programs

  • Strength in administration, organization, and efficiency.
  • Adhere to all deadlines.
  • Complete Child Protection training and adhere to all Child Protection policies and procedures.

Academic and Professional Requirements:

  • Degree in Social Welfare; Master’s level preferred.
  • Diploma in development field, or vast experience or high performance in Family Legacy.
  • Experience in Child Care Facility preferred.
  • Valid GRZ Driver’s License
